I run linphone and sipomatic in parallel and logged the status of the PCM and MIXER device. (see attachment) As you can see there are XRUNS. I played around with the buffer settings in the linphone code and could produce a sound but it's much to quick (rate problem?) and not continious. (buffer problem?) Aplay works okay without any problems. I appended the log, too.
