On this particular system I'm running at kernel 2.4.17 with the
s390-tools-1.1.3 from the May 2002 stream. Shouldn't that version of
dasdfmt already have that patch applied?

This patch contains the following s390-tools bug
fix:

                           Description:
fdasd: missing 390 data set name for
big disks.
                           Symptom:
One of the 390 data set names, which
exists for each partition, is missing for modell 9 DASDs.
                           Problem:
The modell 9 DASD needs a special
freespace label within the VTOC. In this case the "last available FMT1
DSCB"-pointer wasn't set to the right label.
                           Solution:
This pointer is now set correctly.
