This is probably the biggest reason. You're probably getting forced into
the eligible list and your virtual machine is so large that it's never
getting scheduled again.

Break the two functions (mail delivery and POP access) into two
different virtual machines, reduce the size of the virtual machines to
128M, and use VDISK to provide large swap disks. This will drastically
improve performance.

You *really* should upgrade your sendmail. This version has numerous
problems.

Consider switching to exim as well -- if you're doing any real volume,
it's much more efficient than sendmail.
