Two suggestion: 1) find the nearest Xerox demo center and cut a tape with the output. Road trip. 2) use the forms functions in CUPS to print the form and the output simultaneously. You can feed this via RSCS to CUPS and get CUPS to add the form w/o too much trouble. Contact me offline for how to do this.
