It does not require a re-ipl of z/VM to make TCPIP changes effective. You're right that OBEYFILE is temporary - until you change the same thing with another OBEYFILE or you restart TCPIP and have it read the configuration file. So ideally you want to use OBEYFILE to make it work right now, and update the TCPIP PROFILE to have the same setting when TCPIP is restarted some time later. Depending on the type of change, you may want to postpone the update to the TCPIP PROFILE (in case you're not sure, so a restart of TCPIP will undo your damage) or you may need to do it before the OBEYFILE command (when it would not work without the change).
