I would like to make an entry in /dev for a 3270 device.  I added statements in 
the initrd init script to

cd /dev
mkdir 3270
mknode 0770 c 227 0

Why do you want to modify an initrd image ?

If you want to include more kernel modules, check /etc/sysconfig/kernel
under INITRD_MODULES.

Mine looks like :
INITRD_MODULES="ext3 dasd_eckd_mod dasd_fba_mod"
