I setup a test file server with an LVM consisting of 2 3390-9's, which I
then loaded up with files, and it has been working just fine (couple of
months now).
For no reason that I can determine, the LVM has been "lost", resulting in
errors at boot time, thus causing Linux to come up in maintenance mode.

The only thing that I can think of that has happened recently is perhaps
the result of an IPL of VM last weekend?
I had setup the Signal Shutdowntime in System Config to 500 so that the
Linux guests would have time to shutdown, and last weekend was the first
time that I had operations staff shutdown & IPL VM. It seemed to have
worked OK last week as well when I tested it (on Thursday), but then I
didn't notice anything wrong with the Linux file server until this week
(yesterday), so I'm not sure if the "auto" shutdown of the Linux servers
via the VM shutdown is really allowing the servers to come down gracefully
or not - up until last week I had always brought down the servers first,
then shutdown VM. The implied question here is, can a disruptive shutdown
of Linux have an adverse impact on the LVM?

In any case, here's some of the messages from the boot log:

So I am wondering if anybody has any ideas how this might have happened,
and more importantly, how can I go about fixing this?

Any help would be greatly appreciated.
