Try those steps:

1. attach DASDs (e.g. #cp def t3390 <vdev> <cylinders>) to VM guest
2. set DASDs online
3. perform lowlevel format with "-d ldl" option
4. set DASDs offline
5. load "dasd_diag_mod" module
6. set DIAG on for those DASDs

echo 1 > /sys/bus/ccw/devices/0.0.0400/use_diag
echo 1 > /sys/bus/ccw/devices/0.0.0401/use_diag


7. activate DASDs

chccwdev -e 0400,0401


lsdasd

Do I need to do something special to have DIAG mode on ?
Before SLES11 there was some requirements when formatting the disk, but that 
didn't help either.

Does someone have succeeded in having DIAG ON ?

It goes into the udev configuration - in this case a file named
"/etc/udev/rules.d/51-dasd-0.0.0210.rules" should have been created.

I believe YaST takes care of this as well, if you choose to go through YaST,
but I'm not absolutely sure about that one.

I believe that for SLES 11 zipl.conf is no longer used to determine which
dasd to online at boot. Instead, however, you can run `dasd_configure
0.0.0210 1` before you run mkinitrd and zipl, and the specified disk should
be set online and come back online on subsequent boots.
