This question would likely get far more of a response on the IBMVM mailing 
list, but I'll give it a try.  The problem isn't with DTCVSW2, just RDEV 160.  
You should probably check to make sure it's got a cable plugged in, that the 
physical switch sees it, etc.  Perhaps look at the HMC to see if anything there 
shows problems with the card, etc.
