I made a small script to act upon caller ID (send an SMS, etc.). I can send you the scripts if you want. You need no extra software, just a linux supported modem (with a /dev/modem device). I used a MultiTech ISA hardware modem (don't remember the model number).

You'll need to find a CLASS-2 or CLASS-2.0 fax modem, which is quite tricky.
