If you want to rely on Free software, it seems your only choice goes thru a Java-based (ugh, not so Free anymore?) PDF generator called iText:

(I'm not sure you should be worried about performance just because it's Java. From my experience, once those things run, they can offer near-native performance.)


Adobe's commercial solution is Adobe Distiller Server:
http://www.adobe.com/products/acrdis/main.html
I don't know whether it can sign PDFs.
