Check the IRQ on which your card is running.  Also, check if your card is
blacklisted and unsupported by Linux (say a Yamaha YMF-724).

You might (if a Yamaha), try adding these to /etc/conf.modules

alias sound opl3sa2
pre-install sound /sbin/insmod sound dmabuf=1
alias midi opl3
options opl3 io=0x388
options opl3sa2 mss_io=0x530 irq=5 dma=0 dma2=1 mpu_io=0x330 io=0x370

[alter the values to suit your irq, port etc]

Sounds like an unsupported monitor and/or a SiS card :)  See

Adjust the frequencies, mode lines, refresh rates etc in
/etc/X11/XF86config
