This seems to be surely a HDD issue, probably you have suffered powercuts
/ other situational damage to the HDD. Either ways test the HDD by
formatting the partiion with DOS, look for bad sector information.
While installing Linux, if you face such problems, toggle the bad
block check while formatting your partions.
Anaconda is failing for the same reason, methinks......
