its been more than 6 months since i have used lyx so I am bit rusty.
But most likely you cant insert gifs and jpegs. So the way to go would be
to use ImageMagick( aka convert), xv etc to convert them to eps. I heard
theres one converter called jpeg2ps which is supposed to be good.
And about headers and footers you have to go through latex. That is put
an appropriate Latex preamble ( through "layout"-> ). I vaguely remember
some stuff on configuring the default page appearance from
"document"->
custom headers. Look for anything synonymous as i dont remember it well.
