I am not sure of this but I do believe just updating the SMTPROUTES file would do the trick in qmail. It is read by qmail-remote which is spawned for every email requiring a remote delivery.. So I dont think it would need a restart of the qmail system. Even in case qmail needed to re read it's configuration all it is needed is to send a HUP to qmail-send. No need to bring down the SMTP server too :).
