initrd is needed if lilo needs some modules to load the kernel. This is usually needed if you have your kernel on a SCSI disk. You would need the SCSI driver to read from it wont you ;). Otherwise it is usually not needed but never hurts to make on. Man mkinitrd would tell you more about how to do that.
