Check access log of apache. It contains a actual requests sent by browsers. Now
get a line from a windows machine. There are IPs listed too... Telnet to port 80
and type those lines in. See what apache says.
Also check whether there is some info regarding this in error log. Does netscape
on windows work? Or is it IE specific? Or IE is spelling it as INDEX.HTML?
Something liek that...
