first thing you should do is patch all your servers. it's most likely
that you're running something old which was compromised.
ipchains/iptables are not the answer here, you've got to actually stop
services that you don't need. there's no point running services and
then using ipchains to block access to them.
keep up to date with all bug track mailing lists.
and please, please, please, don't say hacked when you really mean
cracked. The two words are totally unrelated, with meanings that do not
intersect anywhere.
