You should have posted the contents of your script.
Anyway you should use something like this:

insmod pctel
insmod ptserial
mknod /dev/ttyS15 c 62 79
chmod 666 /dev/ttyS15
ln -s /dev/ttyS15 /dev/modem

And, things should work.
