Courier IMAP reads the contents of a maildir (or maildir+ in courier mta's case)

qmail when delivering to a user's account for a first time, can create a maildir if it isnt already there.

And when a MUA of any sort (OE, mozilla, squirrelmail etc) connects to an imap server using squirrelmail, it will automatically create folders for itself, assuming the permissions etc are right [for example, uid/gid mismatch between qmail delivery to imap, and imap when you try to create folders / subfolders]

This is not an MTA issue at all
