qmail when delivering to a user's account for a first time, can create a maildir if it isnt already there.

Really? I never knew this. Can you tell how?

use the maildirmake script at any rate - or have the delivery agent (qmail-local) wrapped around something that calls maildirmake if the maildir is not there.

i've seen exim do this automatically ... and this is a delivery agent issue, not an mta issue

I don't get this. Before this, do you mean squirrelmail as the
squirrelmail web interface package or is there any other package called
squirrelmail. How can a MUA like Outlook connect to IMAP server using
squirrelmail, squirrelmail itself is a MUA right?

i mean squirrelmail the webmail mua.

what i mean is both outlook and squirrelmail (as well as any imap capable mail client) will connect to your imap server [courier imap] and create folders within the user's maildir, if the folders they require dont exist.
