Surprised nobody mentioned this one as yet - Big Brother [www.bb4.org]

Have been using it for over a year now and all I can say about this tool is that it is 
simply amazing!

Big Brother provides a framework and tools for monitoring and displaying the current 
status of your servers and entire network and notifying admins as required. It is 
completely extensible and there is a large community of Brothers who have over the 
years written and contributed a wealth of plugins to monitor all sorts of things 
[www.deadcat.net]. Anything you don't find there, but that you can write a script to 
monitor can be incorporated into the system.
