While trying to retrieve the URL: http://www.google.co.in/

The following error was encountered:
Unable to determine IP address from host name for www.google.co.in

The dnsserver returned:
Refused: The name server refuses to perform the specified operation.

This means that:

The cache was not able to resolve the hostname presented in the URL.
Check if the address is correct.

Make sure the nameservers specified in /etc/resolv.conf exist and allow DNS queries from your IP address. Also, in the squid configuration file (/etc/squid/squid.conf) check for a tag named "dns_nameservers". This is used to specify the nameservers to be used by Squid in case you don't want to use the nameservers listed in /etc/resolv.conf. In case you have specified any servers here, make sure that they are accessible by your host.
