Well, you may try autoconfig, but what I did actually was to try every possible
irq and i/o combinations. I was lucky, it worked in third or fourth try. I got
the possible irq and i/o values from isapnpdump, which comes with isapnptools.
The parameters I got is actually different from what w95 uses.

No. You should set uart explicitly with setserial. Most probably it is a 16550A.

setserial is not a tool to test modem, it sets the serial port parameters in the
kernel. After you located the correct values, you should place setserial in your
startup scrips. Minicom was a long way to test the modem, so I just used echo
ATDT>/dev/modem. If this works, most probably your modem will work without any
problems.
By the way, this modem is not a WinModem, huh? Otherwise it won't work.
One last thing about the bios. You should not disable PnP, you should disable
PnP OS installed.
