this might seem like a stupid suggestion, but make sure you're not
replacing the boot disk with the root disk at the wrong time. i think
this is a slack box you're trying to install? the proper sequence is
inserting the boot disk, booting, waiting for the prompt, hitting enter,
then waiting until you get the second prompt, then swapping the boot and
root disks... if that doesn't work, get new disks 'coz they're probably
corrupted.
