I met the IO error you are writing about on my disk. I'm afraid this is a
hardware problem, not exactly bad blocks, but something like that. I had to
repartition my HD in order that all the cilinders with IO errors go to DOS
partitions, so I have rather many of them :-(  And, I'mafraid, this won't
help for long, the HD is slowly dying... Hope, your IO errors are just
accidental. e2fsck finds them and then all works good. :-)
As to fdisk reports -
I can not help, possibly this is the result of HD surface damage, and you
should repartition your disk too - I don't know...
