        If you don't have anything on the floppy you want to keep, try
initializing the floppy with "fdformat /dev/fd0H1440" and then (if you
don't get any errors from the verify) to an "mformat a:".  If you do
get errors, either the floppy drive is dirty or the disk is bad.  I have
had a few occasions where a disk was damaged my magnetic field proximity
and formatting could not recover.  A bulk eraser was able to demagnitize
the floppy and allow it to be formated and used again, but do this only
as a last resort and VERY CAREFULLY (a good bulk tape eraser can penetrate
2" commercial video tape and can take out colateral damage for some
distance).

        I would try cleaning your floppy drive first...  It only takes one
bad (flaking, dirty, finger printed, or stained) floppy to screw your
drive.  Cleaning works wonders.
