try dialing in to your ISP via minicom. There you will be able to see
what the modem says and what it does not like. You can also add the -v
switch to chat and the debug option to pppd and dialin while running
tail -f /var/log/messages (or wherever your syslog resides) in another
xterm.
This should give you a clue.
