Well there are many different modems and ways of connecting things
together and getting them to work.
My provider, zeelandnet supplies a 3com509 ethernet card and a comm21
modem, as to a make i have no idea, it seems that theses days the
mode is dont put a manufactures name on anything, just smoething like
made on China or Taiwan.

It is T-base however and they say it can do 100kb's of which i have
never seen, the highest i have ever gotten is 80.

The MAC address is the ethernet cards personal address, and is in the
form of, HWaddr 00:50:04:A6:FA:C1 which you can see with
'ifconfig'  that is coded into the modem so the cable company knows
its me connecting, which also means if i were to use another computer
with another ethernet card i would need to get the company to change
the mac address in the modem before i  could connect. 

I dont have a static IP# so i have to use DHCP which is very easy to
install and use,
