Thanks for posting this information. After seeing it, I checked on
"Actiontec" in the standard "winmodem" database. It lists several Actiontec
modems, most as unknown, but this one (a PCI modem) as OK:

                    "Actiontec Model
                     PCI56012/PM560LKi; Telepath
                     600/022; IBM Model 33L4618,
                     PCI V.90 56 D/F
                     Controller-based Modem,
                     Lucent Venus chipset"

If this one is no the one you found, it would be a service to the Linux
community to add yours to the listing there. THe URL is
