Since there is no GPL reference in the headers, I do not know if I (or
anyone) can provide you with the file legally. I can tell you that it is
found in the pppd package, which I am sure you can find in any full-size
Linux distribution.
No doubt this package contains the file legally (Linux distributions really
are careful about this), but I do not know what restrictions might apply to
redistribution of the header file independent of the package, particularly
since the overall package is not under the GPL (it's under a different
license from Carnegie-Mellon, as reported in its main.c).
