Update the firmware on your disks. I've had the same happen to a raid 1 with a R200 with a couple of 740GB SATA drives connected to a SAS 6/iR.
The firmware on my drives were not current, and the latest SAS/SATA Hard Drive Firmware Utility (R239489) had a fix mentioned as: Fixes and Enhancements ====================== New firmware for the Seagate SATA hard drives stated below is available that will help improve stability and overall reliability of these drives. During the next scheduled maintenance window, please update the drive firmware to the latest level. In certain rare circumstances, the drive may not return good status after servicing a reset. This fix also enhances performance and addresses improved error handling 1) General stability improvements for heavy IO. 2) Fixes an issue of the activity light on the drive staying off after a firmware download. The 3BKS and below firmware required a drive power cycle to reactivate the activity lights. This is no longer the case with 3BKT firmware. 3) Improved error handling for various forms of reset activities Google "Dell R239489" to find it, and check to see if your disks are affected. Unfortunately Dell only provide drivers for RHEL Update3 (we're now at Update 4), and have advised me that Update 4 drivers may not be available until the new year at the earliest (how it takes ~3 months to release a driver I'm not really sure). LSI, perhaps understandably, refuse to correspond on this matter since this is an OEM board.
