It's been a while, but we did see some problems that sound similar to this.
In our case, there were at least 2 problems.  One was caused, if I remember
correctly, was misconfigured clients requesting ms-chap authentication
causing pppd to hang hard --- upgrade to pppd-2.3.4 to fix that one.  The
other was, again, misbehaving client tcp/ip stacks, which we fixed by
including the following options to pppd to guarantee that we could catch
hung sessions, even if we didn't get the appropriate disconnect or loss of
dcd signals:

lcp-echo-interval 15
lcp-echo-failure 8
