It's been a while, but we did see some problems that sound similar to this. In our case, there were at least 2 problems. One was caused, if I remember correctly, was misconfigured clients requesting ms-chap authentication causing pppd to hang hard --- upgrade to pppd-2.3.4 to fix that one.
