Can't really comment on this problem. I've been using pppd v2.3.7 and
2.3.8 under kernels 2.2.1 and 2.2.9 without any problems. I've just
started experimenting with demand dialing again, so I'll pay more
attention.

There is a README.cbcp in recent 2.3.x ppp source distributions.
Linux can act as a CBCP client with the included patches. Linux PPP
dial-up systems can do callbacks with mgetty options.
