I'm afraid I know basically nothing about hardware RAID controllers, only
about software RAID - but I'll try to help anyway :)

Add to this RAID 1+0 and RAID 0+1.

Isn't hardware RAID usually configured at boot time? Do you get any
messages during initial boot time saying "press F1 to configure the Smart
Array 431" or similar?
I think that h/w RAID presents itself to Linux as one SCSI disk, and that
Linux isn't actually aware of the individual ones.
