Try the beta driver from Qlogic's website:

I had problems with the 2.4.0 qlogic driver too -- this one works fine for
me.
After you extract just do "make qla2x00.o" and "insmod qla2x00.o" -- easier
than their instructions.

I have a Compaq DL360 here and a Qlogic FC card in it.
When I'm trying to boot the machine it will find the
card, and also find the RAID which is connected.
But then it seems to stop. If I then unplug the FC cable
it says that the link is down (which is correct for sure).
If I then replug the cable it will do a Loop ReInit and then proceed
to boot as it should (except that it can't read the partition
table). Anyway, I have enabled some debugging and found,
that it's going through a loop where it does something
for all possible FC devices. It's doing the same loop when I unplug
the FC cable it seems. With one difference - it's doing this
VERY slow. So it's looking like the machine hangs while
it's probably just very slowly going throgh this loop.
I suspect that this is a problem with SMP machines because
I have used the driver before in non-SMP machines with
good success.
I would be very glad if you could provide me with and idea or
maybe even a patch. I can do some testing or further research if
needed.
