In my experience, this cannot come from bad cabling or bad termination.
This can be if the power supply is not enough. Can you try temporarily
with another power supply.

Remember the SCSI specifications: each end of the bus must be actively
terminated. If only using one device on the WIDE connector of the Adaptec
(in or out), set termination to High/Low YES. Also you need to enable the
drive's terminator. Beware that some of the newest drives (especially the
LVD dual-mode models) don't have terminators and you need to terminate
externally (pass through active terminators for example)
Cable length is maximum 1.5 meter. Minimal distance between devices
is 30 cm.
However, I still think you have a power supply problem. Or maybe a
bad drive.
