First of all, your ftape version is prehistoric. Actual
version is 4.03-pre?. Have at look at the ftape homepage and
download actual versions.

I recommend: ftape-4.0?, ftape-tools-1.07 and the
documentation package. Maybe you need actual modutils too.
Check the documentation.

Keep in mind: actual ftape needs kernel compiled without
ftape support and is fully based on modules.

Second, your information is too short to give you further
help. What we should know is: kernel version, module
information and configuration, extract of /var/log/messages.
The latter is the most important because ftape modules send
their error messages to syslogd.
