I can understand an EOT error occurring after writing/before rewind
but why the Self-Diagnostic error?
Is it the case that these errors occurred at power-up but weren't
reported until after the backup completes? That seems a little weird
to me.

I would concur with that BUT ftape went through the motions of
dynamically adjusting it's settings and everything looked ok until
the rewind operation at end-of-backup.
To me this implies that data transfer/DMA operations are functional
so if it's something else (like an internal hardware fault) I'd be
very surprised.
I don't have a Windows machine available and, to be honest, if this
drive comes out, it won't be going back in. For the amount of time
lost in pursuing this and the high cost of Ditto Max media (GPB 25),
perhaps it's time to upgrade to an IDE tape drive.
