Hardware error, probably. Also, "EOT/BOT System Failure" means that
the tape drive failed to detect the holes indicating the end of the
tape. The holes are detected by means of a small LED and something
like a photo-transistor. You can try to clean that equipment. Maybe it
is just dust.
