so this look like a hardware problem.

The "is not formatted" are probably meaningless after that
"Self-Diagostic" error. Does the drive still work with windows? 

The point is: once that "self-diagnostic failed" error is asserted one
cannot do anything more with the drive. Maybe issuing a soft-reset
with "ftmt reset" helps. Probably not. It can be hardware problem,
i.e. maybe your tape drive is broken.

I know that rebooting is ugly, but as this might be a hardware problem
you should a) crosscheck that the thing still works with windows b)
try again after cycling power.
