A hardware defect would be likely. Check that the chip gets powered up
instantly, when you connect the USB power. Also, check that you
followed all the design requirements that FTDI recommends. Finally,
you might want to send me the schematic drawing of your device for
looking at (either in Eagle CAD format, or just as pictures).

These things may influence how soon the chip starts responding to
USB host's requests, so tell me how fast the chip gets recognized by XP.
Windows is known to be very patient with slow devices.

P.S. I used FTDI's FT8U245AM in one of my designs, but I can't recall
how it worked with Linux (the project was Windows only and I had too
little time - as usual - for experiments.)
