FWIW, it was pretty easy to reproduce. Just load the HC driver and mount usbdevfs immediately afterwards: modprobe usb-ohci && mount -t usbdevfs none /proc/bus/usb Didn't take very long to reproduce, atleast in our test case. It may vary from system to system however.
