I still find that very odd.  What could have caused that behavior?  I 
don't think the patch would cause any problems in the lower-level USB 
drivers, and it shouldn't have affected usb-storage adversely either.  
Maybe bad data in the MODE-SENSE input buffer following the error?

Since you're already playing around with the cache detection code in sd.c,
do you think you could try reinstating the patch I sent before and also
put in a long time delay (schedule_timeout()) at the appropriate point in
sd.c so that the system log entries are visible and can get saved?
