I assign them sequentially to patches that I write or adapt. Putting them in the Subject line of emails makes it easy to identify which patch is which and to find them in archive searches. Not all the patches end up getting submitted and they aren't always submitted in numerical order. Also sometimes I have to go back and revise a patch; then I would name it something like as391b, as391c, ...
