It's hard to know just what to say about this, except that these still 
look like low-level interference or hardware errors.  They don't resemble 
the sort of clock timing errors that an external hub can fix.

Either you've had extraordinarily bad luck with choosing your host 
controllers and cables or else you have an incredibly high level of EM 
noise near your system.  :-(

There's always the third possibility, that the Cypress chip isn't working 
right.  Can you try plugging it into that server?
