Do you have sd_mod loaded or in the kernel?

Yep, sd_mod is listed by lsmod ("used by - 0").


The dmesg output after plugging in the device seems to vary. Here's another example:

usb 1-1: new full speed USB device using address 2
usb 1-1: control timeout on ep0in
usb 1-1: unable to read config index 0 descriptor
usb 1-1: can't read configurations, error -110
hub 1-0:1.0: Cannot enable port 1.  Maybe the USB cable is bad?

spurious 8259A interrupt: IRQ7.
usb 1-1: Product: USB Zip 250
usb 1-1: Manufacturer: Iomega
usb 1-1: SerialNumber: 003247B7243A1626
Initializing USB Mass Storage driver...

usb 1-1: USB disconnect, address 3


An extra detail is logged to /var/log/messages, after the serial number output above:

linux /etc/hotplug/usb.agent[6109]: need a device for this command
