I don't know, It is not loading devices correctly. My Logitech Mouseman
wheel mouse isnt even detected when plugged directly into either port.
If I plug in the Belkin USB KVM (with a Logitech iTouch keyboard/mouse
plugged into it), than I get:

Mar 25 00:17:44 phish kernel: usb.c: USB new device connect, assigned
device number 2
Mar 25 00:17:44 phish kernel: usb-uhci.c: interrupt, status 2, frame# 1166
Mar 25 00:17:44 phish kernel: usb.c: USB device not accepting new address
(error=-110)
Mar 25 00:17:44 phish kernel: usb.c: USB new device connect, assigned
device number -1
Mar 25 00:17:44 phish kernel: usb-uhci.c: interrupt, status 2, frame# 1206
Mar 25 00:17:44 phish kernel: usb.c: USB device not accepting new address
(error=-110)
Mar 25 00:17:44 phish kernel: usb.c: USB disconnect on device -1

So I am wondering if the controller is having problems with low-voltage
devices IE the Logitech mouse, and/or it is not handling the KVM's hub
properly or it is not handling the keyboard/mouse receivers correctly.

I guess I am really frustrated and not sure what to do or to try next..
This is with the regular OHLI driver should I compile in the alternate
driver?

This is with the 2.4.3 kernel on a RH7 machine btw.

I am not seeing anything referring to the PIIX3 chip just the
PIIX4 chip that you have, and i don't know what the difference is.
This makes me a litle worried since it is so old.
