I have applied the patch (I assume you are referring to the patch found at http://www.kernel.org/pub/linux/kernel/people/gregkh/usb/2.4/usb-ehci-2.4.20-pre11.patch,If your problem appears in 2.5.latest, or 2.4.20 with that patch, that'd seem to be a new problem, not yet reported by anyone. But a lot of your symptoms sound exactly like ones that have been fixed by that patch.

I am still seeing the same problems as before.Well, it makes things better for many users, evidently not you!
What hardware did you say you had problems with? I got a very
nice email from someone who'd tested with NEC, VT-6202 (PCI and
motherboard), VT-8235, and Intel ICH4, who said the patch made
things a lot better ... except for the VT-6202. And I've had
other reports of problems with that too.
Your 2.4.20-pre11 dmesg info looked fine, but the 2.5.44 one
seemed like it would have been improved by a usb-storage patch
I posted just a smidgeon too late to make that release (should
be in the next 2.5 kernel). And the "after disconnect/reconnect"
looked like you should have tried power cycling the device; some
get into wierd states.
