It looks like your EHCI USB controller isn't working.  Ports 2, 3, and 4 
are unable to reset, and ports 5 - 8 don't hand off to the companions (or 
else maybe the companions aren't working either).

If you rmmod ehci-hcd, so that the EHCI controller isn't being used, do 
the USB connections then start operating?
